Stroke prevention is aimed at controlling or eliminating key risk factors, such as high blood pressure, heart disease, diabetes, high serum cholesterol levels, cigarette smoking, obesity, and physical inactivity. ANTIHISTAMINES Histamine is a chemical mediator found in almost all body tissues. The concentration is highest in the skin, lungs, and gastrointestinal tract. The body releases histamine when exposed to an antigen, such as pollen or insect stings. This results in increased localized blood flow, increased capillary permeability, and swelling of the tissues. In addition, histamine produces contractile action on bronchial smooth muscle. Allergic responses involving histamines and other chemical mediators include local effects such as angioedema, eczema, rhinitis, urticaria, and asthma. Systemic effects from the release of histamine and certain other mediators may result in anaphylaxis see Chapter 33 ; . Antihistamines compete with histamine for receptor sites. Thus they prevent the physiological action of histamine. Table 5 shows the results of the association of persistence with health care related mean GI and non-GI costs by type of service. Mean pharmacy costs incurred by persistent patients were 6.93% higher than those incurred by non-persistent patients P .1 ; . Let me begin with the largest federal purchaser of drugs the Medicare Part D program. Medicare beneficiaries can go on the Medicare.gov web site and find out how much each Part D plan charges for each drug in their formulary. However, the Secretary of HHS, the CMS actuaries, CBO, CRS, GAO, etc. do not know the prices the Part D plans actually pay for these drugs. The data is available, but it has not been analyzed by any of the Congressional agencies or the CMS actuaries. The Secretary of HHS and the Congress should know the prices Part D plans pay for each of the 4300 drugs on one or more of the Part D formularies in order to determine if the Part D market is working. The legislation the House recently passed HR 4 ; will require the Secretary of HHS to know the actual prices that the Part D plans are getting. This is a necessary first step before the Secretary knows where to negotiate. In my opinion, the Secretary does not need to negotiate prices for each of the 4300 drugs. Instead, the Secretary should only negotiate prices where there is market failure and where Part D plans are paying relatively high prices. Let the market work where the market is working effectively; the Secretary should intervene only where there is market failure. This will permit the Secretary to focus attention on the drugs where the market is not working and Part D plans are paying relatively high prices. The Secretary of HHS should compare the lowest price any Part D plan is getting for each drug to the prices that Medicaid, VA or Canada are paying for the same drug. In making this comparison, it is not necessary to report the prices that every Part D plan is paying. All that needs to be reported is the lowest price that any Part D plan could obtain for each drug because this represents the lowest price the marketplace can obtain. The Secretary of HHS can then concentrate his her efforts on negotiating lower prices where there is market failure. The Secretary should compare the lowest price obtained in the marketplace to the VA price because the VA Secretary has negotiated drug prices with pharmaceutical companies. Medicaid prices are an appropriate comparison because this government program has been operating for many years and because Medicaid programs have an extensive formulary. Canada's prices are a relevant comparison because it will show what another country is paying for drugs. Also, if there is a large differential between the Canadian and US prices for drugs, this will cause a substantial number of American seniors to obtain drugs from Canada.

The Food and Drug Administration FDA ; recently announced that it has the authority to force drug manufacturers to switch some of their prescription drugs to o v drugs as a way to make them more affordable and accessible to consumers. In a recent Washington Post article, FDA Commissioner Mark McClellan was quoted as stating that the issue of forcing switches to OTC status is being "actively considered, " and that the FDA feels it has both the legal and the regulatory authority to mandate these switches. The agency is currently considering forcing manufacturers to switch two allergy drugs--Pfizer Corp.'s Zyrtec cetirizine ; and Aventis SA's Allegra fexofenadine ; --to OTC status. Over-the-counter OTC ; drugs are often less expensive than prescription medications but the initial benefits of moving drugs to OTC status usually go to employers and health insurance companies because insurance typically does not pay for OTC drugs. A series of in vitro assays were carried out to determine the effect of PA-457 on the activity of the viral PR enzyme. In a cell-free fluorometric assay using a synthetic peptide substrate PA-457 had no effect on PR function at concentrations of 50 g ml data not shown ; . Experiments using a recombinant form of the Gag precursor protein Pr55Gag and assay formats sensitive to small changes in PR activity gave similar results. In one format, partial Gag processing was achieved by limiting PR concentration, allowing slight changes in enzyme activity to be detected by changes in the relative proportions of the intermediate Gag cleavage products. As shown Fig. 1B ; , using this approach, after 30 min, Gag processing in samples treated with 50 g ml of PA-457 was identical to that observed in untreated controls. The second assay format involved following Pr55Gag processing as a function of time. This approach was particularly sensitive to the effect of test compounds on any single processing step Fig. 6, which is published as supporting information on the PNAS web site ; . In this system, PA-457 exhibited no effect on PR activity. In both experiments, results obtained with PA-457 contrasted dramatically with the results seen with the PR inhibitor indinavir, which blocks all stages of Gag processing.

Prescription of non-sedating antihistamine has increased dramatically between 1996 and 1999 see Figure 6 ; . Among the five drugs, Claritin has clearly dominated the therapeutic class. More recently, however, newer drugs especially Allegra have gained market shares. During the same period, we have observed extensive DTC campaigns to promote these drugs see Figure 7 ; . While Claritin is also the leader in DTC advertising expenditure, this does not appear to have helped increase its market share. In fact regardless of the large amount of DTC advertising, Claritin's market share has declined since 1997. This suggests, once again, that DTC advertising may not have strong effects on physician prescription.
Here is nothing as constant as change! Through a series of events, most unplanned, I have found myself, as have many others working long hours for an extended period of time. There have been many days when all I can do is put one foot in front of the other and move forward wondering if there is light at Dale Dutton, Jane Scurich, Chris Bonfiglio, Patti and Richurd Somers the end of the tunnel. However at the Heart of Marin award luncheon. while driving recently I have come to treasure those moments of solitude ; I took pause to give gratitude and to appreciate the amazing agency that I so fortunate to be a part of. We are doing some truly innovative things with the help of some amazing people the details of which are found in this newsletter. One of our residential homes is being sold. We were given 6 months to vacate a premise that six adults called home for the last 22 years. Two residents would relocate to the Adrian Way Home while four folks and two currently living at Adrian would move into smaller homes and apartments. We began the search for two homes and one apartment the beginning of February. Within one week, we secured rental housing for everyone. Believe me, this is an amazing feat! And the look on the faces of our clients as they become immersed in the process and begin to realize the new found control they will have over their lives is so gratifying. We received a grant to help some of our consumers begin earning income through the vending machine business. We now have vending machines at AngelPoints in Sausalito, the Northgate Mall, and Morgan Stanley. Not only are we on target with the grant, but we are also moving forward with a consumer owned co-op and a move toward employment opportunities for all. There is such a feeling of empowerment and accomplishment when someone is earning income for themselves. The living environments for those adults in our residential homes will soon be much improved. We are in the midst of remodeling the home on Devon Drive and preparing for some necessary maintenance at the home on Adrian Way. On a more personal note, we mourned the loss of one of our residents, Scott Murphy. Scott had been a part of CACS for 19 years. While our feelings of loss continue, we also realize the impact Scott had on those who knew him. We are better people because we were allowed to be a part of Scott's life. Finally, one of our volunteers, Patti Somers was recognized at the Heart of Marin awards in January for her outstanding contribution of volunteer hours during this time of transition, on an ongoing basis and for the larger community. I could not continue to act as executive director of Casa Allegra Community Services without the effort and dedication of all of our volunteers, staff, service directors, management team, and community supporters or without the support of the Board of Directors. On behalf of myself and the people we support I grateful and appreciate you all on a daily basis. We will be faced by the challenges of anticipated budget cuts affecting the lives of those we support and will need your help as we move forward with our mission.
