Tions and a vaccine have shown encouraging results in controlled clinical trials with cocaine-dependent patients. The search for a medical treatment for methamphetamine dependence started more recently, due to the later emergence of this epidemic, but at least one candidate medication has shown promise in early clinical testing. Treatment approaches that combine efficacious medications and empirically proven behavioral interventions, such as voucher-based reinforcement therapy, will almost certainly produce the best results.

What is the most important information I should know if my child is being prescribed an antidepressant? Parents or guardians need to think about 4 important things when their child is prescribed an antidepressant: 1. There is a risk of suicidal thoughts or actions 2. How to try to prevent suicidal thoughts or actions in your child 3. You should watch for certain signs if your child is taking an antidepressant 4. There are benefits and risks when using antidepressants 1. There is a Risk of Suicidal Thoughts or Actions Children and teenagers sometimes think about suicide, and many report trying to kill themselves. Antidepressants increase suicidal thoughts and actions in some children and teenagers. But suicidal thoughts and actions can also be caused by depression, a serious medical condition that is commonly treated with antidepressants. Thinking about killing yourself or trying to kill yourself is called suicidality or being suicidal. A large study combined the results of 24 different studies of children and teenagers with depression or other illnesses. In these studies, patients took either a placebo sugar pill ; or an antidepressant for 1 to 4 months. No one committed suicide in these studies, but some patients became suicidal. On sugar pills, 2 out of every 100 became suicidal. On the antidepressants, 4 out of every 100 patients became suicidal. For some children and teenagers, the risks of suicidal actions may be especially high. These include patients with Bipolar illness sometimes called manic-depressive illness ; A family history of bipolar illness A personal or family history of attempting suicide If any of these are present, make sure you tell your health care provider before your child takes an antidepressant. 2. How to Try to Prevent Suicidal Thoughts and Actions To try to prevent suicidal thoughts and actions in your child, pay close attention to changes in her or his moods or actions, especially if the changes occur suddenly. Other important people in your child's life can help by paying attention as well e.g., your child, brothers and sisters, teachers, and other important people ; . The changes to look out for are listed in Section 3, on what to watch for. Whenever an antidepressant is started or its dose is changed, pay close attention to your child. After starting an antidepressant, your child should generally see his or her health care provider Once a week for the first 4 weeks Every 2 weeks for the next 4 weeks After taking the antidepressant for 12 weeks After 12 weeks, follow your health care provider's advice about how often to come back More often if problems or questions arise see Section 3 ; You should call your child's health care provider between visits if needed. 3. You Should Watch for Certain Signs If Your Child is Taking an Antidepressant Contact your child's health care provider right away if your child exhibits any of the following signs for the first time, or if they seem worse, or worry you, your child, or your child's teacher: Thoughts about suicide or dying Attempts to commit suicide New or worse depression New or worse anxiety Feeling very agitated or restless Panic attacks Difficulty sleeping insomnia ; New or worse irritability Acting aggressive, being angry, or violent Acting on dangerous impulses An extreme increase in activity and talking Other unusual changes in behavior or mood Never let your child stop taking an antidepressant without first talking to his or her health care provider. Stopping an antidepressant suddenly can cause other symptoms. 4. There are Benefits and Risks When Using Antidepressants Antidepressants are used to treat depression and other illnesses. Depression and other illnesses can lead to suicide. In some children and teenagers, treatment with an antidepressant increases suicidal thinking or actions. It is important to discuss all the risks of treating depression and also the risks of not treating it. You and your child should discuss all treatment choices with your health care provider, not just the use of antidepressants. Other side effects can occur with antidepressants see section below ; . Of all the antidepressants, only fluoxetine Prozac ; has been FDA approved to treat pediatric depression. For obsessive compulsive disorder in children and teenagers, FDA has approved only fluoxetine Prozac ; , sertraline Zoloft ; , fluvoxamine, and clomipramine Anafrahil ; . Your health care provider may suggest other antidepressants based on the past experience of your child or other family members. Is this all I need to know if my child is being prescribed an antidepressant? No. This is a warning about the risk for suicidality. Other side effects can occur with antidepressants. Be sure to ask your health care provider to explain all the side effects of the particular drug he or she is prescribing. Also ask about drugs to avoid when taking an antidepressant. Ask your health care provider or pharmacist where to find more information. Prozac is a registered trademark of Eli Lilly and Company. Zoloft is a registered trademark of Pfizer Pharmaceuticals. Anarfanil is a registered trademark of Mallinckrodt Inc. This Medication Guide has been approved by the US Food and Drug Administration for all antidepressants. 2.1 Registration and licensing Foreign companies seeking to do business in Nigeria often prefer to enter the market by making licensing agreements with locally controlled companies rather than establishing their own subsidiaries in the country. This is because of the difficulty of coping with frequent policy shifts which are usually implemented at very short notice ; and the significant overhead costs to maintain a local subsidiary. Licensing is most prevalent among multinational companies in the pharmaceutical sector but is also becoming customary in the high-tech sector where numerous international brands are partnering with locally established firms to penetrate this new and rapidly growing market for information technology and communication products. Multinational companies that have established local subsidiaries or affiliates also enter into licensing agreements to extend the local use of their trade names, trademarks and the manufacture of their products. These include Procter & Gamble US ; , Mercedes Benz Germany ; and Unilever Netherlands UK ; . Notable local licensees include the following: Nigerian Bottling Company, which bottles Coca-Cola, Fanta and Sprite brands of soft drinks under licence from Coca-Cola US SCOA Nigeria, which assembles light pick-up vehicles under a licence from Peugeot France 2.

Background: Data from three randomised controlled trials supports the use of mitoxantrone, a cytotoxic agent, in active relapsing and secondary progressive multiple sclerosis MS ; . The drug has recently been licenced in the United States for "worsening" relapsing remitting and secondary progressive MS. Dosage is limited by cumulative cardiotoxicity to a maximum of 120 mg m2. We report experience from one centre on the use of this agent in patients with aggressive relapsing-remitting MS RRMS ; . Patients: 18 Patients have been treated 11 female ; , mean age 34.6 range 2047 ; , mean disease duration 4.1 years. All patients had experienced two or more disabling relapses with incomplete recovery in the preceding year. Mean Expanded Disability Status Scale EDSS ; prior to treatment was 6.1. All patients underwent regular ECG and Echocardiography for left ventricular ejection fraction. Results: There were no major adverse events, only one patient elected to discontinue treatment at 18 months, disability stable ; . Mean duration of treatment was 17 months. In the 2 years prior to treatment the cohort had 60 steroid treated relapses ARR 1.66 ; , on treatment there were six relapses ARR 0.35 ; . At most recent follow up, or withdrawal of treatment, EDSS was stable + 20.5 points ; in 11 and improved 1 point ; in seven mean EDSS 5.0 ; . Conclusions: Although we would acknowledge that the data we report is uncontrolled; in patients with very active RRMS mitoxantrone seems to be highly effective in suppressing relapse activity. The drug is well tolerated over periods of up to months. Further studies examining the use of Mitoxantrone as an `induction' agent in patients with aggressive disease, prior to the use of standard disease modifying drugs, should be considered and remeron. Subcorneal Pustular Dermatosis SneddonWilkinson Syndrome ; : Another Cutaneous Manifestation of SAPHO Syndrome? SIR--SneddonWilkinson syndrome is a subcorneal pustular dermatosis of unknown pathogenesis sharing histological features with pustular psoriasis, and was originally described in 1956 [1]. It is a clinical entity distinct from other unclassified pustular eruptions and a large number of cases q100 ; have been reported to date. Among rheumatic diseases, it has been described in association with seronegative and seropositive rheumatoid arthritis [26] and systemic lupus erythematosus [7]. We report a case of Sneddon Wilkinson syndrome appearing in a patient with SAPHO syndrome. A 72-yr-old woman was referred from the dermatology unit for pain and swelling of the wrist and metacarpophalangeal MCP ; joints. During the preceding 5 yr she had recurrently developed annular and circinate pustular lesions on the skin of the groins, abdomen, trunk and legs Fig. 1 ; . Biopsy revealed subcorneal pustules without acantholysis, consistent with a diagnosis of SneddonWilkinson syndrome. In the last 3 yr she also suffered cervical and low back pain. On examination, apart from the skin lesions, the sternoclavicular joints were swollen, and the cervical and lumbosacral spine showed a reduced degree of motion. Wrist and MCP joints showed active synovitis, chest expansion measured at the fourth intercostal space was reduced, tenderness was elicited by compression of the sacroiliac joints and modified Shober test was significant. A radiological survey disclosed bilateral sacroiliitis grade 3 on the right, grade 2 on the left ; , randomly distributed syndesmophytes along the spine and arthro-osteitis Fig. 2 ; of the sternoclavicular joints. ESR was 60 mm 1st h, C-reactive protein was 45 mg dl normal Q10 mg dl ; , rheumatoid factor and serology for Klebsiella, Salmonella and Proteus were negative. HLA typing was A2, A29; B44, 35; Cw4, w; DR12, W14; W52, -; DQ1, -. Psoriasis, acne and pustolosis are a group of recognized cutaneous manifestations which may alert clinicians to the presence of a subclinical rheumatic disorder in the setting of asymptomatic or oligosymptomatic patients. Among the pustular lesions associated with joint disease, palmoplantar pustulosis, acne conglobata and hidradenitis suppurativa allow characterization of the SAPHO syndrome, where these skin lesions are generally associated with hyperostosis, osteomyelitis and synovitis [8]. The high prevalence of the B35-Cw4 haplotype in patients with SAPHO syndrome [8] renders B35-Cw4 a genetic marker for this disorder, and supports its diagnosis in our patient. The histopathological aspects of the pustules in SAPHO syndrome and in SneddonWilkinson syndrome are identical, except for their localization: subepidermal in the former case, subcorneal in the latter. To date, a subcorneal pustulosis has never been described in association with arthro-osteitis of the sternoclavicular joints and bilateral sacroiliitis, all. Wal-Mart has grown steadily through its focus on price leadership but the company also continuously responds to the needs of customers in new regions, and in retailing categories not yet fully exploited. Novartis has emerged as a crucial partner in one such category: carrying a more direct health-care message to Wal-Mart customers. Over the past 18 months, more than 450 000 Wal-Mart shoppers have received free blood-pressure screenings through nationwide "Healthy Heart" events co-sponsored by Novartis and Wal-Mart. Disease information about hypertension available at the "Healthy Heart" events raised awareness about the "silent killer" that affects an estimated 60 million Americans. When shoppers discover that their blood pressure is above the norm, and that they need to see a doctor and get it checked, they are likely to remember Wal-Mart as a catalyst of treatment.
