In a small number of patients treated with AVANDIA adverse experiences of oedema, hypercholesterolaemia and anaemia decreased haemoglobin ; were reported in double blind trials. Oedema Oedema was reported in 4.8% of patients taking AVANDIA, compared to 1.3% on placebo, 1.0% on sulfonylureas and 2.2% on metformin. Treatment was required for 1.2% of patients with an adverse event of oedema. These adverse experiences infrequently led to withdrawal. Oedema was generally dose-related and was more frequently observed when rosiglitazone was used in combination with a sulfonylurea or insulin. Oedema was more frequently observed when rosiglitazone was used in combination with insulin compared with insulin alone 14.7% vs 8.2% ; and occurred with a greater frequency in those patients on the higher 8 mg daily dose of rosiglitazone. Metabolism and Nutrition Disorders Hypercholesterolaemia was reported in 3.4% of patients. The elevated total cholesterol levels were associated with an increase in both LDLc n 2048 ; and HDLc n 2177 ; and the ratio of total cholesterol: HDLc was unchanged or decreased in long term studies n 886 after 12 months' therapy ; . Overall, these experiences were generally mild to moderate and usually did not require.
Avandia is the brand name for a medication used to manage type 2 diabetes. It was approved by the Food and Drug Administration FDA ; in 1999 and one million Americans are currently taking it. It can be taken once or twice a day. A person with type 2 diabetes shows high levels of blood sugar because their body does not respond to insulin, a hormone released by the pancreas. Avandia makes it possible for the insulin the body produces to move glucose out of the bloodstream and into the cells where it is used for energy. This week the news media announced that persons taking Avandia have an increased risk of 43 percent for having a heart attack. They also published a 64 percent increased risk of dying from cardiovascular-related events while taking the drug. These results are the conclusion of an evaluation of 42 studies comparing patients taking the drug with those not taking Avandia. Overall, there were 86 heart attacks and 39 deaths among 15, 560 Avandia patients. This compared with 72 heart attacks and 22 deaths among 12, 283 patients not taking Avandia. FDA officials acknowledge the potential increased risk for those taking Avandia, but also noteworthy is the risk for side effects if those same persons change medications or do not keep blood sugar levels controlled. Experts caution people not to stop taking the medication on their own, but to talk to their provider to further discuss their options. It might prove to be useful for identifying areas that may be precancerous and need to be biopsied. Wireless Capsule Endoscopy. Wireless capsule endoscopy WCE ; is a new imaging approach that may be very useful for Crohn's disease. With WCE, the patient swallows a capsule containing a tiny camera that records and transmits images of the gastrointestinal tract as it passes through. Some studies have found it to be more accurate in evaluating small bowel disease than the use of barium x-rays or CT scans. Patients also accept it better than standard endoscopy. It has some limitations, however. For example, it is sometimes difficult to interpret nonspecific findings. At this point, it is useful only for problems in the small intestine. It is also not clear if this advance will add any benefits beyond those achieved with standard endoscopy. Ultrasound. Intestinal wall ultrasound is proving to be useful for identifying the extent and severity of Crohn's disease. Although it is unclear if ultrasound is useful for an initial diagnosis, one study indicated that it is effective for identifying Crohn's disease or ulcerative colitis when used by experienced professionals. Upper and Lower Gastrointestinal Barium X-Rays. An upper gastrointestinal barium x-ray may be used if Crohn's disease is suspected in the small intestine. Swallowed barium passes into the small intestine and shows up on an x-ray image, which may reveal inflammation, ulcers, and other abnormalities.

INDICATIONS AND USAGE AVANDIA is indicated as an adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus. AVANDIA is indicated as monotherapy. AVANDIA is also indicated for use in combination with a sulfonylurea, metformin, or insulin when diet, exercise, and a single agent do not result in adequate glycemic control. For patients inadequately controlled with a maximum dose of a sulfonylurea or metformin, AVANDIA should be added to, rather than substituted for, a sulfonylurea or metformin. AVANDIA is also indicated for use in combination with a sulfonylurea plus metformin when diet, exercise, and both agents do not result in adequate glycemic control. Management of type 2 diabetes should include diet control. Caloric restriction, weight loss, and exercise are essential for the proper treatment of the diabetic patient because they help improve insulin sensitivity. This is important not only in the primary treatment of type 2 diabetes, but also in maintaining the efficacy of drug therapy. Prior to initiation of therapy with AVANDIA, secondary causes of poor glycemic control, e.g., infection, should be investigated and treated. CONTRAINDICATIONS AVANDIA is contraindicated in patients with known hypersensitivity to this product or any of its components.
The hormone insulin helps cells to absorb sugar from the blood. A consequence of aging and, in some cases, the use of HAART is that cells may become less sensitive to the effects of insulin, a condition called insulin resistance. If left untreated, insulin resistance can get worse and lead to the development of diabetes. To help re-sensitize cells to the effects of insulin, dietary changes, an exercise program and quitting smoking are all important steps to take. If these are not enough, then insulin-sensitizing agents are necessary. These include metformin Glucophage ; and a class of drugs called glitazonesrosiglitazone Avandia ; and pioglitazone Actos ; . Glitazones, particularly rosiglitazone, are popular among some doctors who treat PHAs. Several years ago there were high hopes that glitazones would also be able to reverse body shape changes seen with the HIV lipodystrophy syndrome. However, these drugs have failed to do so large clinical trials. Although rosiglitazone is popular, it is not well known how this drug affects the processing in the liver of other drugs taken by PHAs. German researchers in Dusseldorf and Berlin studied some of the interactions between anti-HIV agents and rosiglitazone. Choice of speaker for the keynote address. Many colleagues from around the world have fed back that they regarded the Very Reverend Michael Mayne's address as the most inspiring moment during their time in Oxford. Several people regretted missing this event. Michael Mayne's words appeared to draw together and touch the core of people from all different traditions, systems of belief and philosophy and experiences. His final reminder to consider a person as 'not a problem to be solved but as a mystery to be loved' was a powerful message for us all, coming from a man whose life has been dedicated to ministering to and serving others. Again this collection presented here enables us all to reflect further on the more ineffable aspects of our work, areas that have always been part of the work but perhaps, post-Oxford, have been given a more coherent voice. I have many treasured memories of the 10th World Congress of Music Therapy. I recall the variety of meetings and discussions with many old and new colleagues and friends from around the world. There was a real lively feeling of Dialogue and Debate, even if Friday evening's programmed debate on the survival of music therapy surprised a few by not being perhaps as formal as expected! If you happened to be present at this rather outrageous event you may recall that, in my rle as Chair, I chose to conclude by over-ruling the arguments for the survival of music therapy. However as I mentioned in the closing ceremony the quality of the work presented at Oxford and now available in this format bears witness to the fact that there is no fear of music therapy not surviving well into this century.
