Reminyl ER is one of a group of drugs called "cholinesterase inhibitors" which is used to treat symptoms in people with mild to moderate Alzheimer's disease. In the brains of people with Alzheimer's disease there is a progressive degeneration of nerve cells, particularly of the cells that make acetylcholine, a chemical thought to be important for learning and memory. Because of this, people with Alzheimer's disease have lower brain levels of acetylcholine. It is believed that Reminyl ER works by reducing the breakdown of acetylcholine and thus increasing the amount of the chemical in the brain. It has also been suggested, based on data from animal experiments, that the medication enhances the action of acetylcholine by making the receptors it interacts with in the brain more responsive. In the region of the brain first affected by Alzheimer's disease, that dealing with cognition and memory, too little acetylcholine is available at the junctions between nerve cells to get messages across to the next nerve cell, The situation is helped, therefore, not only by preserving the acetylcholine from being destroyed by cholinesterase, but by making the receptors more responsive to the lower amounts of acetylcholine. Whether this effect is also true for humans is unknown. The potential beneficial effect of Reminyl ER could lessen as the disease progresses and when fewer cells are available to make acetylcholine. Ongoing research is finding that combining cholinesterase inhibitors such as Reminyl ER, Aricept or Exeon ; together with memantine a drug used to relieve the symptoms of people with moderate to severe Alzheimer's disease ; seems to greatly improve outcomes, sometimes more than predicted from the sum of the effects of either drug alone. However, more and larger drug trials are needed to confirm these promising early results.
In 2002, the Susquehanna River Basin Commission the Commission ; convened the Conowingo Pond Workgroup the Workgroup ; to recommend a management plan for the Conowingo pond, a 14 mile-long interstate water body created by construction of the Conowingo dam on the Lower Susquehanna River. The pond, which straddles the Pennsylvania-Maryland border, serves multiple uses. During recent low flow conditions on the Susquehanna River, the pond demonstrated an inability to meet all existing uses, and the Commission had determined that a more comprehensive management scheme was needed to avoid conflicts. As a regional, interstate agency with basinwide water allocation and consumptive use regulatory authority, the Commission was uniquely qualified to initiate and lead the Workgroup effort. The Workgroup undertook a four-year planning effort to evaluate operational alternatives for the pond and to recommend to the Commission a management plan that best meets the water use needs identified by the Workgroup. Additionally, the Workgroup was tasked with identifying management actions that the Commission should incorporate into its regulatory and water resource management programs. The Workgroup completed their report in March 2006, which served as the basis for the Conowingo Pond Management Plan adopted by the Commission. The composition of the Workgroup was intended to represent the interests of key stakeholders in the operation and use of the pond. Participation remained open to any interested party throughout the process, but invitations to participate were extended directly to targeted representatives from federal and state agencies, local jurisdictions, operators of the lower Susquehanna hydroelectric facilities and Peach Bottom Atomic Power Station, local water utilities, and the Commission. Importantly, all targeted parties were active in Workgroup activities. The Workgroup met several times a year and provided direction, oversight, input, and review for the planning effort and its results. Other interested parties that did not directly participate were kept apprised of the Workgroup's progress. As noted above, the Conowingo pond was created by the construction of the Conowingo dam in 1928 to provide hydroelectric power generation for the Conowingo Hydroelectric Station. Effect of Exelon on the Metabolism of Other Drugs: Rivastigmine is primarily metabolized through hydrolysis by esterases. Minimal metabolism occurs via the major cytochrome P450 isoenzymes. Based on in vitro studies, no pharmacokinetic drug interactions with drugs metabolized by the following isoenzyme systems are expected: CYP1A2, CYP2D6, CYP3A4 5, CYP2E1, CYP2C9, CYP2C8, or CYP2C19. Dementia is an age-related problem that increases Medicaid expenditures by increasing the number of nursing home placements and by raising prescription drug and other health care costs. Alzheimer's disease AD ; is considered to be one of the most debilitating of diseases, impairing the ability of affected individuals to live independently and reducing their quality of life. In the state of Florida there are currently about 420, 000 older adults living with the disease, a significant increase from the 370, 000 estimated to have had AD in 2001 OPPAGA, 2001 ; . Most persons with AD and other dementias 70%-80% ; live in the community and receive most of their long-term care from family and friends Dwyer, Barton, & Vogel, 1994 ; . However, as dementia progresses, cognitive and functional decline and deterioration lead to an increase in the risk of nursing home placement Cummings et al., 2002; Friedman, Steinwachs, Rathouz, Burton, & Mukamel, 2005; Heyman, Peterson, Fillenbaum, & Pieper, 1997 ; . Results from some studies indicate that giving ARVs to an individual very early in the course of their HIV infection correlates with better control of the virus over the long term because it helps spare the immune system from some of the damage inflicted early on by the virus. Though maintaining a strong immune system requires a balanced diet and a complete intake of all the important nutrients, certain vitamins and minerals have a particularly strong immune-enhancing effect. Vitamin C, for example, enhances the white blood cell response and function. By stimulating natural killer cell activity and boosting the blood levels of major antibodies, vitamin C, when taken in adequate amounts, helps keep the troops of the immune system in tip-top shape.38 Numerous clinical studies have shown its effectiveness in treating the common cold, as well as other infectious conditions.39 A common partner of vitamin C, vitamin E has also shown strong immune-enhancing properties.
